Requesting Medical Records

The Health Information Management department maintains records of the care patients receive while they are at Temecula Valley Hospital. As required by state and federal law, these records are kept in strict confidence and are only released with proper authorization from the patient. We offer the following options described below to obtain copies of a patient's medical records and/or radiology study from Temecula Valley Hospital:

In-Person

As we continue our efforts to combat the effects of COVID-19, the safety of our patients and staff is of upmost concern. Therefore, we are not allowing in-person access to our Release of Information department, with the one exception noted below. Please use the other options provided on this website to request access to your records, such as the Patient Portal, online, mail, phone and/or your provider. We apologize for any inconvenience.

Exception for In-person Requests: Process servers and patients/representatives who require urgent/same-day imaging CDs who are not exhibiting any COVID-19 related symptoms.

The Release of Information department is located at 25485 Medical Center Drive, Suite 208, Murrieta, CA 92562. The department is open from 8:30 am to 4 pm, Monday through Friday, excluding holidays.

Note: While you are at your provider's office for your follow-up appointment, the provider's office can call the Release of Information department (951-696-6013, option 2) for any required documentation and we will fax the information directly to their office. In addition, imaging CDs can be mailed directly to your provider. Please request a few days prior to your appointment to allow enough time for mail to be received.

Please be advised that to protect our patients' privacy and health information, we do not accept telephone requests and we do not disclose any information over the telephone.

Radiology Images and Studies

Radiology images and studies require one full business day to prepare. If you are scheduling follow-up appointments and will need these materials, please plan accordingly.

Medical Record Delivery Options

After we have processed your request for medical records, we will mail the information to the address specified on your request form, or you may pick up your copy from our office. Due to the large volume of requests we receive, it may take up to 5 business days to fulfill your request from the date we receive it.
